Medical records technicians may be needed to record the vast amount of information collected for each patient's medical history. Technicians may keep these all patients' charts up-to-date within a practice or hospital by ensuring all forms are complete and properly identified, and that all necessary authentications have been added or updated for physicians and other healthcare professionals. In addition to organizing patients' physical charts, medical records technicians may also add this information to an electronic database.

Keeping detailed and accurate patient records is an important aspect in the healthcare industry. The task of a medical records technician may be to manage the database and conduct quality assurance measures to ensure the validity of these records, which may include:
- Treatment plans
- Patient medical history
- Test results
- Descriptions of symptoms
Medical records technicians may be responsible for managing the patient's records from the beginning until the end of their treatment. They may need to ask patients to fill out appropriate forms and ensure that these are properly signed. In addition, they may need to communicate with physicians and other healthcare practitioners in a consistent basis to make sure that all information recorded is complete.
There are many diagnosis-related groups (DRG) which medical records technician may use to classify patients according to their diagnosis. By assigning specific codes to each diagnosis or medical procedures, the medical records technician is able to determine how much the clinic or the hospital should be reimbursed by companies using the DRG systems such as Medicare. Long-term healthcare and emergency services have their own coding system.
Computer systems allow technicians to retrieve large amounts of data to provide patient care, research, and information in the case of lawsuits. The responsibility of a medical records technician is largely dependent on the institution where they work. For example, small medical facilities hire experienced technicians to supervise the whole records department.
